All IMG's and medical students from schools outside the United States must apply through the Educational Commission for Foreign Medical Graduates ( www.ecfmg.org ) for the USMLE steps 1, 2 CK and 2 CS.

    •  Obtain an USMLE/ECFMG number which will be your identification for any USMLE regarding process. Access the ECFMG webpage and enter the IWA section. If you are a first time user, you must register and access your personal information such as your name, email, address, home country, medical school, and year of graduation. The system will verify your data and a few days later will create your USMLE/ECFMG identification number. You'll receive an email notification in which your USMLE/ECFMG identification number will be included.

    •  Access the IWA section of the ECFMG webpage using your USMLE/ECFMG identification number. Select the option that will allow you to apply to the exam you want (USMLE steps 1, 2 CK, or 2 CS). Complete the on-line application filling the required information. Be sure to use proper upper and lower cases when required, and to carefully write all your information. Your name must be written in Latin characters and must coincide exactly with the name in your passport or other government-issued credential. Information concerning your medical school will be required (address, phone and fax number, curriculum, credits per subject, etc.) so have it at hand when you start your application. Also, when applying to the USMLE step 1 or step 2 CK, you will be asked where you intend to take it, since for these two tests only, overseas locations are available. You will have the opportunity to review all your information and proofread it before submitting the application.

    •  After submitting your application, print the Certification of Identification and the Authorization Forms. You will need to adhere one full face photo in the designated space of the Certification of Identification, and take it to a medical school official who will verify your signature and stamp the form and your photo. The Authorization Forms authorize the medical school to release your information for verification purposes; you must sign it and put it together with the Certification of Identification. The Certification of Identification will be required only for processing your first exam application; further applications will be processed automatically.

    •  You must pay the examination fee in order for your application to be processed. On-line payment option with credit cards is encounter while filling your on-line application. If you want to pay with check or money order, a Fee Payment Form must be printed and send it together with the rest of the documentation. All payments must be done in American Dollars.

Send all documentation (Certification of Identification, Authorization Forms, a copy of your medical diploma if graduated, Form 183 if a medical student, and Fee Payment Form if applicable) through certificated mail or courier to the ECFMG mailing address. ECFMG will process your application and will send to your medical school the copy of your medical diploma together with a questionnaire to be filled by your Dean. The Dean must verify your information, certify that it is accurate and then send it back to ECFMG. If no inconvenient is encounter during the verification process, a Scheduling Permit will be mailed to you. The Scheduling Permit is a red paper document of extreme importance. It is only with this document that you will be able to reserve a day for your exam. In the case of the USMLE step 2 CS, you may print the permit (referred as Confirmation Notice) from the IWA section once the process is completed and your appointment is made.
